Benefits

The following benefits may be covered.

• Vision Exam - Vision plans may provide a wellness eye exam, less co-pay, designed to provide an eye refraction for vision and a early screening for medical conditions.
  (Note: A wellness vision exam covered by your vision plan does not provide coverage for injury or one or more medical conditions. In the case of injury or one or more medical conditions your visit will be processed through your medical insurance.)
• Contact lens - benefits for contact lenses are outlined by your plan.
• Glasses - benefits for glasses are outlined by your plan.

Authorization

Before your vision exam, AEA administrative staff will discuss your vision plan with you and contact your Vision Plan Insurance Company to receive authorization for your:
